True Link Honors Recipient of Grant For Best Practices in Person-Centered Support for 2024

For more than a decade True Link has empowered professionals who support their clients’ independence and well-being. As a part of that mission, True Link offers an annual grant to support those actively working to promote standards, collaboration, transparency, and robust systems of oversight in person-centered support. It’s awarded to field-builders — those advancing the field through the implementation and dissemination of best practices that support guardians, trustees, and the individuals they work to serve day in and day out. 

We are pleased to announce that Jennifer Case has been selected as the recipient of our 2024 Grant for Best Practices in Person-Centered Support!

Jennifer Case has dedicated over 23 years to SCARC Guardianship Services, Inc., serving as both the Director of their Pro Se Legal Advocacy Program and a Senior Guardianship Caseworker. During her time at SCARC, a few of her accomplishments include:

-Helping Nearly 2,000 Families Obtain Guardianship

Jennifer has directed the Pro Se Legal Advocacy Program at SCARC, successfully assisted nearly 2,000 families in Northern New Jersey in obtaining guardianship for their adult children with intellectual/developmental disabilities. She provides essential education and guidance on legal processes, ensuring families can navigate the complex system without the need for costly help. This highlights her dedication to making guardianship accessible for those who might otherwise be unable to afford it.--

-Advocating for Clients and Promoted Person-Centered Support

As a Senior Guardianship Caseworker and National Certified Guardian, Jennifer has consistently championed the rights and expanded the opportunities of countless clients with developmental disabilities. Her expertise in person-centered support ensures that every guardianship is tailored to the individual's unique needs, focusing on their wishes and empowering guardians to help their family members live their best lives. She collaborates with various professionals and trains others on important topics like identifying abuse or neglect, demonstrating her comprehensive approach to client well-being.

-Expanding Outreach and Accessibility of Guardianship Information

Jennifer has been instrumental in the outreach component of the Pro Se program for two decades. Under her leadership, the program's reach has continually grown. Notably, she successfully transitioned some presentations to virtual webinars, which more than doubled attendance. This has made it significantly easier for families to access crucial guardianship information without the need to hire a caregiver for their child, showcasing her commitment to increasing accessibility.

“Working at a small non-profit has often meant that we find ways to make a big difference on a small budget. This grant will significantly enhance both our Pro Se and Guardianship programs. The funding will allow us to expand access to critical legal resources for families seeking guardianship for their family member with special needs, ensuring they can navigate the court system with greater confidence and clarity.
Additionally, it will strengthen our guardianship program by enabling more comprehensive training, oversight, and support for guardians, ultimately protecting the rights and well-being of vulnerable people with intellectual disabilities.”

Jennifer is an exceptionally dedicated and empathetic advocate for individuals with intellectual/developmental disabilities as proof through her decades of service guiding families and those with disabilities. She is a highly respected expert in her field and her unwavering commitment to improving the lives of those she serves is deserving of recognition.

With this grant, True Link hopes to amplify the voices of those who are leading the field to achieve its highest aspirations for person-centered support, helping promote transparency and personal decision making, and enhancing the lives of individuals every day.
